Vaccination Exception Request

If you have a documented disability or a medical or religious exemption for which you can provide proof, please email info@eduwebsummit.com. eduWeb has a confidential review committee to review all requests and documentation. Please download and complete an Accommodation or Exemption Request Form and email your request for exemption, along with supporting documents.

Exemption review and decision will be made within 3 business days. All exemption requests and documentation must be emailed by July 22, 2022. It is recommended that attendees finalize registration after receiving exemption approval. If attendee registration is finalized and an exemption request is not approved, attendees may request a full refund and it will be provided. Check refunds will be issued within 30 days of the request, and credit card refunds will be processed within three business days.

When submitting an Accommodation or Exemption Request Form:

  1. Enter your personal information.

  2. Select one of the three reasons (as listed below).

  3. Email documentation.

  4. Type your name in the signature box to confirm the information entered is true and accurate. 

Disability Accommodation

In accordance with the Americans with Disabilities Act (ADA), eduWeb Summit provides reasonable accommodations, absent undue hardship, to qualified individuals with disabilities. If you believe you need accommodation because of a disability, you are responsible for requesting a reasonable accommodation by submitting an Accommodation or Exemption Request Form. Supporting documentation will be required and treated as personal and confidential.   

Medical Reasons

Exemptions are available for medical reasons that make receipt of the vaccine dangerous or otherwise inappropriate. This may also include conditions such as history of certain allergic reactions and any other medical condition that is a contraindication to the COVID-19 vaccine, even if it does not qualify as a disability under federal, state, or local law. If you believe you need an accommodation regarding this policy for medical reasons, you are responsible for submitting an Accommodation or Exemption Request Form. Supporting documentation will be required and treated as personal and confidential.

Religious Accommodation

eduWeb Summit provides reasonable accommodations, absent undue hardship, to employees with sincerely held religious beliefs, observances, or practices that conflict with getting vaccinated. If you believe you need an accommodation regarding this policy because of your sincerely held religious belief, you are responsible for submitting an Accommodation or Exemption Request Form. You will be required to explain your sincerely held belief and provide documentation from your religious leader or published literature regarding the religious belief that conflicts with eduWeb Summit's vaccination requirement.
